To make your first visit as smooth as possible, we ask all new patients to complete their forms online before their appointment.
Once your appointment is scheduled, you will receive secure access to two important tools:
- Patient Portal – Your personalized portal will allow you to complete medical history forms, update information, and review documents related to your care.
- Detailed instructions on our Patient Portal.
- Klara Messaging – Through Klara, you can securely communicate with our team and receive reminders or instructions about your upcoming visit.
How to Access Your Forms
- You will automatically receive an email and/or text invitation with login details for both the Patient Portal and Klara after scheduling your first appointment.
- Please complete your forms in the Patient Portal before your visit. If you have any questions or difficulty accessing your portal, our staff will be happy to assist. Please arrive 15-20 minutes before your appointment to complete these forms in the office.
